By signing this agreement, I acknowledge that I have read, understood, and agree to the following terms and conditions:

1. Family Companion

I understand that this dog is being adopted as a family companion and not for neglect or abandonment. The dog will live indoors, sleep inside the home, and will never be left outside without proper supervision.

2. Food and Water

I agree to provide my dog with fresh, clean water at all times and feed a high-quality, nutritious diet appropriate for its age and health.

3. Veterinary Care

I agree to provide regular veterinary care throughout the dog’s life, including but not limited to:

• Annual wellness examinations

• Vaccinations as recommended by my veterinarian

• Rabies vaccinations as required by local or state law

• Heartworm prevention

• Flea and tick prevention

• Prompt medical treatment for any illness, injury, or emergency

4. Health Certificate

I understand that this puppy has been examined by a licensed veterinarian before adoption and has received all age-appropriate vaccinations and health care. A health record and health certificate will be provided at pickup. While every effort has been made to ensure the puppy is healthy at the time of adoption, Shadow Rose Mini Dachshunds cannot guarantee the future health, physical condition, size, or development of the puppy after it leaves her care.

5. Future Medical Expenses

I understand that health issues may arise after adoption, including illnesses or injuries requiring emergency treatment or surgery. I accept full responsibility for all veterinary expenses after I take ownership of the puppy.

6. Licensing and Identification

I agree to obtain any licenses required by my local or state authorities and ensure my dog always wears appropriate identification, including a rabies tag, license tag (where required), and personal ID tag attached to a safe collar.

7. Follow-Up Contact

I understand that Shadow Rose Mini Dachshunds may contact me from time to time to check on the puppy’s well-being. I agree to respond to phone calls, text messages, or emails in a timely manner and cooperate with reasonable follow-up requests.

8. Lost Dog

If my dog becomes lost or missing, I agree to notify Shadow Rose Mini Dachshunds immediately.

9. Unable to Keep the Dog

If I am no longer able to care for the dog, I agree to contact Shadow Rose Mini Dachshunds immediately and offer her the first opportunity to assist with rehoming or taking the dog back. I agree to provide at least 30 days’ notice whenever possible. During this period, I remain responsible for the dog’s care and any boarding expenses if necessary. I understand that Shadow Rose Mini Dachshunds is not obligated to accept the return of the dog but will make every reasonable effort to assist.

10. Behavioral Issues

I agree to notify Shadow Rose Mini Dachshunds of any serious behavioral concerns, including aggression or biting. If requested, I agree to seek an evaluation from a qualified professional dog trainer or behavior specialist.

11. Liability

Once I take possession of the dog, I accept full responsibility for its care, behavior, and actions. I release Shadow Rose Mini Dachshunds, her family, and representatives from any liability, claims, damages, injuries, losses, or expenses arising from my ownership of the dog.

12. Medical Records

I understand that all available veterinary records, vaccination records, and health documentation will be provided at pickup. I acknowledge that the puppy is adopted based on the information available at the time of adoption and that Shadow Rose Mini Dachshunds makes no guarantees regarding the puppy’s future health, temperament, training, breeding ability, or adult characteristics.

13. Adoption Fee

I understand that all adoption fees are refundable, except where otherwise stated in writing by Shadow Rose Mini Dachshunds.

14. Breach of Agreement

I understand that failure to comply with any of the terms outlined in this agreement may result in legal action and, where permitted by law and the terms of this agreement, may require the return of the dog to Shadow Rose Mini Dachshunds.

15. Legal Agreement

By voluntarily signing this agreement, I acknowledge that I have carefully read and understood all of the above terms and that I am entering into a legally binding agreement with Shadow Rose Mini Dachshunds.
